Through the Million Hearts campaign, private and public partners, including public health departments and Medicaid agencies, are working to prevent one million heart attacks and strokes by 2017. A number of states are using an interagency partnership approach across Medicaid and public health to enhance cardiovascular disease (CVD) prevention and treatment. This fact sheet, which was developed in partnership with the Association of State and Territorial Health Officials, explores Vermont and Oklahoma’s interagency partnership to address hypertension as part of the Million Hearts campaign. It also highlights key considerations for states when pursuing this type of partnership to address CVD.
